Open since 1996, our family practice moved to a brand new building in 2005, conveniently located on the east side of Martinsville, off Route 40. David Davis, MD, John Richards, MD, Theresa Hershfield, FNP, and Rachel Morrison, PAC, are here for all your family practice needs. They can also refer you to specialists as needed. Four visiting physicians from the UAP clinic in Terre Haute see patients at the practice twice a month. Appointments need to be made directly through the UAP clinic at the following numbers:

Clark County Family Medicine provides medical services including lab and x-ray for the entire family. The lab is open during the practice opening hours.

Open since 2009 in Martinsville, our locally owned and family operated pharmacy is conveniently located under the same roof as Clark County Family Medicine. The pharmacy offers a local delivery service and a drive-through window. They fill prescriptions, compound medicine and sell medical equipment. A pharmacist is always on staff to provide personalized service.

The Clark County Health Department offers food sanitation classes, immunizations, as well as a Women, Infants & Children (WIC) program.
